Effectiveness of Structured Teaching Programme on Knowledge regarding Fast Hugs Bid in improving Patients Care and Safety of Critically Ill Patients among Staff Nurses working in Icu Set-Ups in Selected Hospitals

ABSTRACT:

A pre-experimental study to assess the effectiveness of structured teaching programme on knowledge regarding FAST HUGS BID in improving patients care and safety of critically ill patients among staff nurses working in ICU set-ups in selected hospitals at Rajkot. The objectives of the study were (a) To assess the knowledge regarding FAST HUGS BID among staff nurses working in ICU set-up. (b) To assess the effectiveness of structured teaching programme on knowledge regarding FAST HUGS BID in improving patient care and safety of critically ill patients among staff nurses working in ICU set-ups. (c) To find out the association between post test knowledge score with their selected demographic variables.1, The research approach selected for the study was quantitative research approach and pre-experimental research – one group pre-test post-test design. The study was conducted at selected hospital at Rajkot. Total 40 samples were selected from ICU settings based on non-probability purposive sampling. The researcher used structured interview schedule and structured questionnaire for data collection regarding demographic variables and pre-test. As a part of intervention structured teaching programme on FAST HUG BIDS was carried out among staff nurses working in the ICU setting. The post-test score was measured with the same questionnaires after 7 days. The collected data were analyzed by using inferential statistical method. t- test was used to evaluate the effectiveness of STP on the level of knowledge regarding FAST HUG BIDS among ICU nurses. It revealed that the mean score of pre test was 6.63 and posttest was 15.33. The Mean difference was 8.7. The obtained ‘t’ value 18.91. Hence it was highly significant p<0.05 level. Hence their findings of the study revealed that the Structured Teaching Programme was effective in improving knowledge regarding FAST HUGS BID in improving patient care and safety of critically ill patients among staff nurses working in ICU set-ups.

INTRODUCTION:

Health is primary of all the functions of human being. In humans health it is the ability of individuals or communities to adapt and self-manage when facing physical, mental or social challenges. An individual with healthy body and mind is an asset to the family and society. But health can be destroy by various disorders or diseases. Disease broadly refers to any condition that impairs the normal functioning of the body. Some minor disease can be treated at home itself, while some sever diseases requires medical therapy or surgical treatments in the health care set-up.2

 

Clients with critical condition are requires to be observed in intensive care unit of hospital setting. Intensive care unit is a special department of hospital that provides intensive treatment medicine to the patient with severe and life-threatening illnesses and injuries. For improvement of quality of care some new innovative treatments modalities are always being introduced in the ICU setting. For the improvement of the communication among nursing and physician caregivers in the ICU, one tool has been introduced a FAST HUGS BID.3

 

NEED FOR THE STUDY:

Medical errors are relatively common and variation in practice exists between clinicians, even when best-practice guidelines are available. Because of lack of awareness of hospital staffs some of the major complications are getting arise in the ICU. Important complications of care in ICU consists of infections including ventilator-associated pneumonia, catheter- associated blood stream infections and urinary infections; venous thromboembolism, delirium, myopathies and neuropathies related to critical illness, stress ulcers, decline in functional status and ICU acquired weakness.4

 

So hospital provider specially ICU nurses should be aware of the major complications of the ICU care in order to implement evidence – based preventative measures when possible, to screen patients for complications after an ICU stay and to inform conversations with patients and families about goal of care.5

 

Open communication between all members of ICU is essential to avoid complication and to conduct quality improvement of care aimed at reducing complications. Recognizing a need for improved communication among nursing and physician caregivers in the Medical Intensive Care Unit, leadership on the unit introduced a tool- FAST HUGS BID. It has been suggested that the use of checklists in ICU may be effective in improving patient care and safety

 

OBJECTIVES:

1    To assess the knowledge regarding FAST HUGS BID among staff nurses working in ICU set-up.

2    To assess the effectiveness of structured teaching programme on knowledge regarding FAST HUGS BID in improving patient care and safety of critically ill patients among staff nurses working in ICU set-ups.

3    To find out the association between post test knowledge score with their selected demographic variables.

 

HYPOTHESES:

H1: There will be significant improvement in knowledge regarding FAST HUGS BID among staff nurses working in ICU set-ups after structured teaching programme.

H2:  There will be significant association between the knowledge regarding FAST HUGS BID in staff nurses working in ICU set-ups with their selected demographic variables.

 

MATERIAL AND METHODOLOGY:

Research approach:

Quantitative research approach

 

Research design:

Pre- experimental, one group pre-test post-test design

 

Setting:

The study was conducted in Synergy Hospital at Rajkot.

 

Population:

Accessible population:

Staff nurses working in Synergy Hospital at Rajkot

 

Target population:

Staff nurses working in the ICU set-ups in synergy hospital at Rajkot.

 

Sample:

Sample size:

The sample size of the study was 40 staff nurses working in ICU set-ups.

 

Sampling technique:

The research study was conducted by non-probability purposive sampling technique.

 

Data analysis plan:

The collected data was analyzed using both descriptive and inferential statistics.

 

RESULTS:

Major study findings includes

 

A. Findings related to demographic variables of the study:

1    The majority of 26 (65%) sample’s age is between 51-65 years

2    The majority of 25 (62.5%) samples are male.

3    The majority of 23 (57.5%) samples are having education of GNM.

4    The majority of 14 (35%) samples are working in medical ICU.

5    The majority of 22 (55%) samples are having 1-5 years of work experience.

6    The majority of 21 (52.5%) samples are having previous knowledge about FAST HUGS BID and it’s importance of use.

7    The majority of 13 (32.5%) samples are having in-service education programme as a source of information about FAST HUGS BID.

 

With regard to effectiveness of STP on knowledge regarding FAST HUGS BID among ICU staff nurses, the obtained ‘t’ value for the level of knowledge was 22.74 that was highly significant at p<0.001 level.

 

With regard to association between the level of knowledge with their selected demographic variables such as Educational qualification, Previous knowledge and Source of information had significant association found. The study shows that the calculated Chi-square value is more than the tabulated value at the level of 0.05 for these demographic variables. This shows there is a significant association between the effectiveness of STP and knowledge regarding FAST HUGS BID and selected demographic variables.

 

CONCLUSION:

The main conclusion from this present study is that most of the staff nurses working in ICU had inadequate and moderate level of knowledge in pre-test and they improved to moderate and adequate level of knowledge in post-test. This shows the imperative need to understand the purpose of the STP regarding improving the knowledge about FAST HUGS BID in improving patients care and safety of critically ill patients among ICU staff nurses and it will improve the practice of FAST HUGS BID mnemonic to improve quality care of nurse.
